Resumo: Introdução: A comunicação é uma competência essencial para o(a) médico(a) e outras categorias profissionais, e deve ser desenvolvida durante sua formação profissional. A elaboração de um projeto de comunicação, incluindo um consenso brasileiro, visou subsidiar as escolas médicas a preparar os estudantes de Medicina para se comunicarem efetivamente com os(as) cidadãos/cidadãs brasileiros(as), de características plurais intra e inter-regionais, pautando-se no profissionalismo e nos princípios do Sistema Único de Saúde (SUS). Objetivo: Este manuscrito apresenta o consenso para o ensino de comunicação nas escolas médicas brasileiras. Método: O consenso foi construído colaborativamente com 276 participantes, experts em comunicação, docentes, profissionais de saúde e discentes, de 126 escolas médicas e cinco instituições de saúde, ao longo de nove encontros presenciais em congressos e de encontros virtuais quinzenais ou mensais. Nos encontros, compartilharam-se as experiências dos participantes e o material bibliográfico, incluindo os consensos internacionais, e apresentou-se o consenso em construção, com discussão em grupos para elencar novos componentes para o consenso brasileiro, seguida por debate com todos para pactuá-los. A versão final foi aprovada em reunião virtual, com convite a todos(as) os(as) participantes em julho de 2021. Após submissão, diversas alterações foram requeridas, o que demandou novos encontros para revisão da versão final do consenso. Resultado: O consenso tem como pressupostos que a comunicação deve ser centrada nas relações, pautada nos princípios do SUS, na participação social e no profissionalismo, e embasada nas Diretrizes Curriculares Nacionais do curso de graduação em Medicina, em referenciais teóricos e nas evidências científicas. São descritos objetivos específicos para desenvolver a competência em comunicação nos estudantes, abrangendo: fundamentos teóricos; busca e avaliação crítica da literatura; elaboração e redação de documentos; comunicação intrapessoal e interpessoal no ambiente acadêmico-científico, na atenção à saúde em diversos contextos clínicos e na gestão em saúde. Recomenda-se a inserção curricular da comunicação do início ao final do curso, integrada a outros conteúdos e áreas de saber. Conclusão: Espera-se que esse consenso contribua para a revisão ou implementação da comunicação nos currículos das escolas médicas brasileiras.


Abstract: Introduction: Communication is an essential competence for the physician and other professional categories, and must be developed their professional training. The creation of a communication project including a Brazilian consensus aimed to subsidize medical schools in preparing medical students to communicate effectively with Brazilian citizens, with plural intra and inter-regional characteristics, based on the professionalism and the Brazilian Unified System (SUS) principles. Objective: The objective of this manuscript is to present the consensus for the teaching of communication in Brazilian medical schools. Method: The consensus was built collaboratively with 276 participants, experts in communication, faculty, health professionals and students from 126 medical schools and five health institutions in face-to-face conference meetings and biweekly or monthly virtual meetings. In the meetings, the participants' experiences and bibliographic material were shared, including international consensuses, and the consensus under construction was presented, with group discussion to list new components for the Brazilian consensus, followed by debate with everyone, to agree on them. The final version was approved in a virtual meeting with invitation to all participants in July 2021. After the submission, several changes were required, which demanded new meetings to review the consensus final version. Result: The consensus is based on assumptions that communication should be relationship-centered, embedded on professionalism, grounded on the SUS principles and social participation, and based on the National Guidelines for the undergraduate medical course, theoretical references and scientific evidence. Specific objectives to develop communication competence in the students are described, covering: theoretical foundations; literature search and its critical evaluation; documents drafting and editing; intrapersonal and interpersonal communication in the academicscientific environment, in health care and in health management; and, communication in diverse clinical contexts. The inclusion of communication in the curriculum is recommended from the beginning to the end of the course, integrated with other contents and areas of knowledge. Conclusion: It is expected that this consensus contributes the review or implementation of communication in Brazilian medical schools' curricula.

ABSTRACT

OBJECTIVE: To investigate the association between parental bonding styles and anxiety, depression, suicidal ideation, and self-efficacy beliefs in undergraduate medical students. METHODS: A cross-sectional, self-administered survey involving 315 Brazilian medical students was conducted online. The Parental Bonding Instrument (PBI), the Generalized Anxiety Disorder-7 (GAD-7) scale, the 9-item Patient Health Questionnaire (PHQ-9), the Suicidal Behaviors Questionnaire-Revised (SBQ-R), and the general self-efficacy (GSE) scale were used. The internal consistency of the instruments used in the study was analyzed using Cronbach's alpha. Multiple logistic regression models were applied, and the odds ratios (OR) and respective 95% confidence intervals (CI) were calculated to determine the association between parental bonding styles and anxiety, depression, suicidal ideation, and general self-efficacy beliefs. RESULTS: In the analysis adjusted for sociodemographic variables, maternal affectionless control was associated with a greater risk of anxiety (OR = 2.48; 95% CI: 1.15-5.33), depression (OR = 7.54; 95% CI: 3.20-17.78), suicidal ideation (OR = 3.62; 95% CI: 1.58-8.27), and low self-efficacy (OR = 3.81; 95% CI: 1.76-8.25), while maternal neglectful parenting was associated with depression (OR = 3.24; 95% CI: 1.17-8.96) and paternal affectionate constraint with suicidal ideation (OR = 3.09; 95% CI: 1.36-7.02). CONCLUSIONS: These findings showed dysfunctional parenting styles to be associated with mental illnesses and low self-efficacy in Brazilian undergraduate medical students. This should be taken into consideration when treating medical students with mental disorders.

Resumo: Introdução: Neste artigo, fazemos um relato de experiência da implantação e do funcionamento de um programa de mentoria aplicado a estudantes de graduação do curso de Medicina de uma universidade do Centro-Oeste brasileiro. É consenso que a pressão dos cursos de Medicina provoca sobrecarga emocional e afeta negativamente os estudantes, e é preciso que medidas de apoio sejam implementadas. Relato de experiência: O programa de mentoria da Faculdade de Medicina da Universidade Federal de Goiás teve um processo de construção lógico. Foi criado no início de 2015, pelo reconhecimento da necessidade da instituição em apoiar os acadêmicos durante sua graduação, fato evidenciado a partir do atendimento psicológico ao aluno e de uma pesquisa in loco. Os seguintes aspectos-chave caracterizam o programa: é oferecido como uma disciplina eletiva, pode ser feito até cinco vezes e,além de um encontro mensal com o mentor, o estudante escolhe as oficinas de que deseja participar, diversificando assim sua formação. Após 12 semestres de funcionamento, sintetizamos neste artigo uma parte de nossos resultados e algumas reflexões sobre o que queremos para o futuro do programa. Discussão: A implementação da mentoria e suas adequações foram resultados de pesquisa, capacitação, discussões dos docentes e avaliação periódica pelos mentorados. As mudanças procuraram atender às expectativas e sugestões dos estudantes, objetivando atrair a atenção e satisfazer aos anseios dos discentes. Ao longo dos 12 semestres de funcionamento do programa, o interesse dos alunos pela matrícula na disciplina cresceu, com forte avaliação positiva, especialmente pela livre escolha e diversidade de oficinas, e pelo envolvimento do tutor. Conclusão: As instituições de ensino devem estimular a criação de programas de mentoria. O programa relatado neste artigo tem tido boa aceitação por parte dos alunos e nos aponta bons resultados.


Abstract: Introduction: This article reports on the experience of implementing and running a mentoring program for medical school students at a Brazilian university in the Midwest region. It is widely accepted that the pressure at medical school is detrimental to the students and often leads to emotional overload. Therefore, supporting mechanisms must be implemented. Experience report: The mentoring program at the Federal University of Goiás had a logical beginning. It was created in early 2015, after psychotherapy services offered to the students led to an in loco research study, which in turn pointed out the need to provide support to students throughout their undergraduate studies. There are some key characteristic aspects of the program: it's an elective course, which can be taken up to 5 times, and the students can choose the workshops they want to attend in addition to the monthly meeting with the mentor, which allows for diverse paths to be followed. After twelve semesters in existence, this article presents a summary of some of the results obtained and reflections on the future of this program. Discussion: The implementation of the program and its adaptations resulted from research, professional development, discussions among professors and regular assessments by the mentees. Changes were made according to the expectations and suggestions of participating students, aiming to effectively meet their needs and represent a desirable course. Since its inception, the program has attracted increasingly more students, and enjoyed a strong positive evaluation, mainly due to the various workshops to choose from and engagement with the mentor. Conclusion: Universities should support the creation of such mentoring programs. The program discussed in this article has been well received and points towards positive outcomes.

Abstract: Introduction: Multiprofessional health residents deal with high stress daily, harming patient care, which can cause emotional and interpersonal reactions that contribute to the development of Burnout Syndrome, thus disclosing the importance of identifying the at-risk conditions for the syndrome and possible protection. The objectives of this study were to describe the occurrence of burnout, its impairment dimensions and the association with sociodemographic characteristics in multiprofessional residency professionals. Method: This is a descriptive research with a quantitative approach, conducted with multiprofessional health residents from public hospitals located in a capital of the Midwest region of Brazil. The ISB - Burnout Syndrome Inventory - was employed, using six indices (Positive Organizational Conditions, Negative Organizational Conditions, Emotional Exhaustion, Emotional Distancing, Dehumanization and Professional Achievement), associated with the assessment of sociodemographic characteristics such as gender, marital status, profession, previous work, change of city, living alone, having children, getting another degree, enjoying what you do in your free time, practicing physical activity, having a religion, undergoing a therapeutic process. The logistic regression method was used to verify the association between these characteristics and the diagnosis of burnout syndrome. A total of 134 residents participated in the survey. Results: The results showed that Emotional Exhaustion is present in 91% of participants; Emotional Distance in 89.6%; Dehumanization, in 61.9%; Professional Achievement in 11.2%; Positive Organizational Conditions, in 85.1%; and Negative Organizational Conditions, n 82.1%. A positive association was observed between taking simultaneous courses, male gender and living alone. Conclusion: It is observed that the practice of therapy can reduce the chance of developing the syndrome and it is concluded that there is a high prevalence of burnout syndrome in the assessed group, with it being a response to stress defined by the presence of exhaustion, distancing and dehumanization. Despite these results, the perception of professional achievement remains independent of stress.


Resumo: Introdução: Os residentes multiprofissionais da saúde lidam diariamente com alto nível de estresse, favorecendo o prejuízo ao atendimento do paciente, o que pode causar reações emocionais e interpessoais que contribuem para o desenvolvimento da síndrome de burnout, revelando, assim, a importância de identificar as condições de risco para a síndrome e a possível proteção. O objetivo deste estudo foi descrever a ocorrência de burnout, suas dimensões de comprometimento e a associação com características sociodemográficas entre profissionais da residência multiprofissional. Método: Trata-se de uma pesquisa descritiva, com abordagem quantitativa, realizada com residentes multiprofissionais em saúde de hospitais públicos localizados em uma capital do Centro-Oeste do Brasil. Adotou-se o Inventário da Síndrome de Burnout (ISB), utilizando-se de seis índices (condições organizacionais positivas, condições organizacionais negativas, exaustão emocional, distanciamento emocional, desumanização e realização profissional), associados à avaliação de características sociodemográficas, como sexo, estado civil, profissão, trabalho anterior, mudança de cidade, morar sozinho, ter filhos, fazer outro curso, gostar do que faz nas horas vagas, praticar atividade física, ter religião, estar em processo terapêutico. O método de regressão logística foi utilizado para verificar a associação entre essas características e o diagnóstico da síndrome de burnout. Participaram da pesquisa 134 residentes. Resultados: Evidenciou-se que a exaustão emocional está presente em 91% dos participantes; o distanciamento emocional em 89,6%; a desumanização em 61,9%; a realização profissional em 11,2%; as condições organizacionais positivas em 85,1%; e as condições organizacionais negativas em 82,1%. A associação positiva foi feita entre a realização de cursos simultâneos, a característica sexo masculino e o fato de morar sozinho. Conclusões: Observa-se que a prática de terapia pode reduzir a chance do desenvolvimento da síndrome e conclui-se que há uma alta prevalência da síndrome de burnout no grupo pesquisado, sendo resposta ao estresse definido pela presença de exaustão, distanciamento e desumanização. Apesar desses resultados, a percepção de realização profissional se mantém independente do estresse.

ABSTRACT

BACKGROUND: The frequently observed stress of medical students worldwide leads them to have psychic suffering often leading to illness. Minor psychic disorders such as anxiety, depression and burnout, have a higher prevalence in these students than in the rest of random population. Different initiatives were tried to minimize the deleterious effects of the medical course and this article aims at showing the repercussions of a elective course that and was proposed as a possibility to help the students. METHODS: A qualitative case study took place in a public Brazilian university as an elective discipline offered to medical students in 2013, offering coping strategies for professional stress. The data was collected through a semi-structured individual questionnaire that was anonymous, and given to students on the last day of the course, with 18 Likert scale questions about personal and behavioural changes observed after taking the course. Objective questions were asked about their perception of stress at the beginning and at the end of the course: the use of the coping strategies taught and the perception of the utility of the content. In addition, one open-ended question was asked about the meaning of the discipline to the students. The quantitative data was analysed with descriptive simple statistics and the qualitative with the support of the WebQDA software. The research project was approved by the ethics committee of the institution. RESULTS: The results showed that the course contributed positively to the students' academic life: 67% reported less symptoms of stress at the end of the course; 76% adopted new coping strategies; and 90% considered that this learning activity was useful for identifying stressors and sharing them with colleagues. CONCLUSIONS: The elective course produced benefits to the students, representing theoretical-practical learning and an opportunity for reflection and self-knowledge, which caused psychological, behavioural and lifestyle changes. It is recommended that further studies on this theme should be conducted.

ABSTRACT

Este trabalho é um relato de experiência do primeiro ano de funcionamento do Grupo de Apoio ao Primeiranista (GAP). O GAP é um grupo desenvolvido por alunos de Medicina de uma universidade privada do Estado de São Paulo, apoiado por sua direção, com o intuito de amparar e humanizar a recepção aos ingressantes. O início das atividades do GAP ocorreu em 2013 com base nos conceitos de tutoria/mentoring, aplicado por outras escolas médicas brasileiras. Nas reuniões semanais do GAP, os participantes (alunos do primeiro ano) e mentores (acadêmicos veteranos e uma psicóloga institucional) discutiam temas como resiliência, compaixão, humildade e respeito. Durante as reuniões, notou-se uma problemática extensa em relação ao trote universitário, referido pelos primeiranistas como o pior problema enfrentado na universidade. Na sequência dos encontros, o grupo direcionou suas ações para reduzir o trote na universidade. O GAP, apesar de estar em seu primeiro ano de funcionamento, mostrou ter eficácia relevante, podendo ser um modelo a ser implementado em outras universidades.


This paper reports the experience of the first year of the Freshman Support Group (GAP). GAP was developed by a group of medical students at a private university in the state of Sao Paulo, supported by their coordinators, with the aim of supporting and humanizing the reception of new first-year students. GAP opened in 2013 founded on the concepts of mentoring applied at other Brazilian medical schools. GAP held weekly meetings in which participants (first year medical students), and mentors (both senior students and a psychologist), discussed topics such as resilience, compassion, humility, and respect. During the meetings the students voiced much concern about university hazing; cited as the worst problem faced at university. Following these meetings, the group directed its actions toward reducing hazing at the university. GAP, although only in its first year of operation, proved to be effective and could serve as a model for implementation at other universities.

BACKGROUND: The undergraduate medical course is a period full of stressors, which may contribute to the high prevalence of mental disorders among students and a decrease in life's quality. Research shows that interventions during an undergraduate course can reduce stress levels. The aim of this paper is to evaluate the Strategies for Coping with Professional Stress class offered to medical students of the Federal University of Goiás, at Goiânia, Goiás, in Brazil. METHODS: Qualitative research, developed with medical students in an elective class addressing strategies for coping with stress after a focal group (composed of nine of the 33 students taking this course) identified stress factors in the medical course and the coping strategies that these students use. Analysis of the results of the class evaluation questionnaire filled out by the students on the last day of class. RESULTS: Stress factors identified by students in the focus group: lack of time, excessive class content, tests, demanding too much of themselves, overload of extracurricular activities, competitiveness among students and family problems. Coping strategies mentioned in the focus group: respecting one's limits, setting priorities, avoiding comparisons, leisure activities (movies, literature, sports, meeting with friends and family). Results of the questionnaires: class content that was considered most important: quality of life, strategies for coping with stress, stress factors, assertiveness, community therapy, relaxation, cognitive restructuring, career choice, breathing, social networking, taking care of the caregiver, music therapy and narcissism. Most popular methodologies: relaxation practice, drawing words and discussion them in a group, community therapy, music therapy, simulated jury, short texts and discussion. Meaning of the class: asking questions and reinforcing already known strategies (22.6%), moment of reflection and self-assessment (19.4%), new interest and a worthwhile experience (19.4%), improvement in quality of life (16.1%), expression's opportunity (9.7%), other (6.4%). CONCLUSION: The stressors perceived by the medical students are intense and diverse, and the coping strategies used by them are wide-ranging. Most students felt that the class was a worthwhile learning experience, incorporated new practices for improving quality of life and recognized the importance of sharing and reflecting on one's stressors and life choices.

ABSTRACT

Este estudo objetivou analisar as percepções de usuários de fitoterapia quanto às vantagens e desvantagens decorrentes da sua utilização e influência na qualidade de vida. Constitui-se em pesquisa qualitativa, desenvolvida entre maio e julho de 2006, junto a 22 usuários de um hospital especializado em terapias alternativas, localizado em Goiânia-GO, Brasil. Para análise dos dados, utilizou-se o referencial da análise de conteúdo. Os usuários referiram que, antes de iniciarem a terapêutica fitoterápica, buscaram tratamentos alopáticos, entretanto, não obtiveram sucesso. As vantagens decorrentes da utilização da fitoterapia apontadas pelos sujeitos relacionam-se à eficácia, ausência de efeitos colaterais, baixo custo e estímulo aos hábitos saudáveis de vida. Entre as contribuições para melhoria na qualidade de vida, destacou-se a harmonia familiar, maior capacidade para realizar atividades do cotidiano e o sentimento de bem-estar. A fitoterapia parece proporcionar mudanças significativas na vida de seus usuários e contribuir para melhorias na qualidade de vida.


The aim of this qualitative study was to examine user perceptions of the advantages and disadvantages of phytotherapy and its influence on quality of life. Twenty-two users at a hospital specializing in alternative therapies, in Goiânia, Goiás State, Brazil, were studied from May to July 2006. Data analysis was framed by content analysis. Users said that before starting phytotherapy they had sought allopathic treatment, which was unsuccessful. As benefits of phytotherapy, users mentioned its effectiveness, freedom from side-effects, low cost and encouragement of healthy living habits. Prominent among its contributions to improving quality of life were: harmony in the family, increased ability to perform everyday activities, and a sense of well-being. Phytotherapy appears to produce significant changes in users’ lives and to contribute to improving their quality of life.


Este estudio objetivó analizar las percepciones de usuarios de fitoterapia cuanto a las ventajas y desventajas de su uso y su influencia en la calidad de vida. Se constituye en pesquisa cualitativa, desarrollada entre mayo e julio de 2006 junto a 22 usuarios de un hospital especializado en terapias alternativas, situado en Goiânia-GO, Brasil. Para el análisis de los datos, se usó el referencial del análisis de contenido. Los usuarios refirieron que, antes de iniciar el tratamiento fitoterápico, han buscado tratamiento vinculado a la alopatía, sin embargo, no tuvieron éxito. Los beneficios de la utilización de la fitoterapia apuntados por los sujetos están relacionados con: la eficacia, la ausencia de efectos secundarios, el bajo costo y el fomento de hábitos saludables de vida. Entre las contribuciones para la mejora de la calidad de vida han destacado: la armonía familiar, una mayor capacidad para realizar actividades de la vida cotidiana y el sentido de bienestar. La fitoterapia parece proporcionar cambios significativos en la vida de sus usuarios, contribuyendo para mejoría de la calidad de vida.

ABSTRACT

INTRODUÇÃO: O presente estudo teve como objetivo investigar a prevalência de sintomas depressivos em alunos de medicina da Universidade Federal de Goiás, comparando com a população em geral e verificando a possibilidade de determinado período do curso atuar como fator de risco. MÉTODO: Estudo transversal, descritivo, de amostra representativa de alunos regularmente matriculados na Faculdade de Medicina da Universidade Federal de Goiás. Foram selecionados 287 alunos do primeiro ao sexto ano através de sorteio aleatório simples. Utilizou-se, para avaliação, o Inventário de Depressão de Beck e um questionário de identificação. RESULTADOS: Na população entrevistada, 26,8 por cento apresentaram sintomas depressivos de acordo com escores do Inventário de Depressão de Beck estabelecidos para este estudo. A prevalência de sintomas depressivos moderados e graves entre os entrevistados foi de 6,9 por cento, enquanto 19,9 por cento apresentaram sintomas leves. Em relação ao sexo, encontrou-se 33,5 por cento de mulheres e 19 por cento de homens com sintomas depressivos. Em relação ao ano em curso, houve maior prevalência entre os alunos do terceiro e do quarto ano. Tristeza, anedonia, baixa auto-estima, perfeccionismo, irritabilidade, desinteresse por pessoas, redução da capacidade de trabalho e cansaço excessivo foram os itens do Inventário de Depressão de Beck mais pontuados. CONCLUSÃO: A prevalência de sintomas depressivos encontrada neste estudo é superior à média encontrada na população em geral, havendo indicativo de que a escola médica possa ser um fator predisponente para tais sintomas.


INTRODUCTION: The objective of the present study was to assess the prevalence of depressive symptoms among medical students of Universidade Federal de Goiás, Brazil, comparing them to the general population and investigating the possibility of a certain period throughout the course being a risk factor. METHOD: A cross-sectional descriptive study of a representative sample of students regularly enrolled at the Medical School of Universidade Federal de Goiás was performed. A total of 287 students from the first to sixth year were selected using simple random drawing. The Beck Depression Inventory and an identification questionnaire were used for evaluation. RESULTS: In the interviewed population, 26.8 percent had depressive symptoms according to the Beck Depression Inventory scores established for this study. The prevalence of moderate and severe depressive symptoms among the interviewees was 6.9 percent, while 19.9 percent had mild symptoms. As to gender, 33.5 percent of the women and 19 percent of the men had depressive symptoms. There was higher prevalence between third- and fourth-year students. Sadness, anhedonia, low self-esteem, perfectionism, irritability, lack of interest for people, reduced work capacity, and excessive tiredness were the most punctuated items of the Beck Depression Inventory. CONCLUSION: The prevalence of depressive symptoms found in this study is higher than the average found in the general population, and there is an indication that the medical school can be one of the predisposing factors for these symptoms.
